- Can the University allowance for insurance be used to purchase outside insurance plans?
- No. The University allowance for insurance may only be used to purchase University-sponsored benefits.
- What plan elections does the University allowance cover?
- For the employee only, the University allowance pays for Medical Plan A, Dental Plan B, and Vision Plan A.
- When may dependents be dropped from coverage?
- Dependents may be dropped at any point during the year, effective the end of the month in which the change is made.
- When may dependents be added to insurance coverage?
- Dependents may be added during the annual open enrollment period or when a qualifying event occurs. Qualifying events include marriage, divorce, birth, loss of other credible coverage, and other situations.
- What happens to unused sick leave at retirement or termination of employment?
- Unused earned sick leave is reported to MOSERS for all employees eligible under the MSEP or MSEP 2000. For every 168 hours of unused, earned sick leave, the employee receives an additional month of service credit. Unearned time is not reported to MOSERS and does not earn additional service credit. Unused leave does not allow an employee to retire sooner than his or her first eligible date.

- A listing of the prescription drugs covered at various co-payments by the University's insurance program is available at http://www.anthem.com/ select Providers on the left hand side of the webpage. If you require additional assistance, please contact the Benefits Coordinator.
- Can you tell me if my contributions toward my IRA qualify for the IRA deduction (line 32) on my federal income tax? I have contributed $3600 this year, as indicated on line 12a of my W-2 form. Thanks!
- Line 12a on the University W-2 actually represents contributions to a qualified tax-sheltered 403(b), not contributions to an Individual Retirement Account (IRA). Beyond this clarification, the University cannot provide information or recommendations regarding the completion of an individual’s tax returns. We recommend contacting your financial planner or tax professional.
- My son graduated from college in May. When does his medical coverage (as my dependent) expire?
- Insurance coverage for dependents who graduate in May of a given year lasts through August 31 of that same year.
- How can I get another copy of my Anthem BC/BS Card? It went through the wash.
- New Cards can be ordered at http://www.anthem.com/ if you sign up for your own User ID and password; or contact human resources and we will order the card for you which will be sent to your home address.
- Our daughter who is a full-time college student is thinking of marrying next summer. Can we continue to carry her on our medical insurance even though married? If yes, can we carry her through grad school?
- For all lines of coverage (medical, dental, vision and life insurance), qualifying dependents include the following: spouses and unmarried children under the age of 19 or unmarried children under the age of 25 who are enrolled full-time at an accredited institution. Unmarried children must be able to be claimed as dependents for income tax purposes.
- Can I get information about survivorship arrangements as related to my retirement investments and life insurance?
- Information concerning MOSERS or CURP concerning retirement may be found by contacting representatives at (800) 827-1063 or mosers@mosers.org.
- Does the HSA option give access to the prescription drug plan?
If so, can the copay come from the HSA account?

- I have not received a dental insurance card. I would like to have an appointment for the dentist this week. Would you please give me a suggestion?
- Contact the Benefits Coordinator and have a new card ordered.
- I have resigned from my position which will go into effect on May 31st. How long do I have insurance coverage?
- Your medical, dental and vision care benefits end May 31, 2008; you will be offered continuation of benefits under COBRA guidelines. Please contact HR for more information.
- My wife and I have just adopted a 17 month old girl. What do I need to do to have her added to my insurance?
- Please contact the Benefits Coordinator and change your enrollment form. Please bring in a copy of the adoption papers for your file.
- I would like to get new glasses. How do I go about using my vision insurance since I do not have a card with my policy number (as I have never received one)?
- Please contact the Benefits Coordinator for information on securing new frames and glasses.
- My daughter graduates in May and turns 25 in June. Can she continue health insurance through all of June?
- Dependents are covered on the Anthem plan to the end of the calendar year in which they turn 25.
